We have an exciting opportunity for enthusiastic andmotivated Occupational Therapists to join our highly regarded Urgent CommunityResponse Service, based at Woking Community Hospital, which provides responsiveNHS community services to patients across North West Surrey.
Although expected to work autonomously you will not bealone; you will be supported by the Occupational Therapy Team Lead. The teamalso includes Physiotherapists, Nurses, Frailty Practitioners, Senior CommunityRehabilitation Assistants and Administrators; and works closely with colleaguesfrom GP practices, locality frailty hubs, adult social care and other communityservices.
As a key partner in the delivery ofresponsive services to support the frail population, the service is keen tosupport the development of team members to enhance their skills and broadentheir scope of practice where appropriate.

Main duties of the job
Working as a member of the multi-disciplinary team youwill undertake holistic assessments, clinical diagnosis, and treatment ofpatients in their own home, who may require crisis management and/orrehabilitation interventions to prevent acute hospital admissions, supportcomplex hospital discharges and maximise their independence.
The role will include the triage andface to face assessment of patients on the Urgent Community Response, Falls andtherapy pathways, and often the assessment and provision of equipment tosupport safe mobility and activities of daily living. As a result of yourassessment, you may also arrange short term support provided by our SeniorCommunity Rehab Assistants, and liaise with colleagues in local GP practices,locality frailty hubs, adult social care teams, and other community services.

Communication / Relationship skills
To communicate effectively with all disciplinesinvolved in a patients care, both those from within the organisation and thoseoutside, to ensure effective multidisciplinary working. This will includediscussion of patient care, patient progress and involvement in dischargeplanning.
To be able to communicate complex and sensitiveinformation to patients, carer and other staff including imparting unwelcomenews relating to their rehabilitation potential and gaining consent fortreatment.
To communicate any changes in care plan ortreatment approach to other relevant members of the Urgent Community ResponseTeam and theUrgent CommunityResponse Operational Lead as necessary.
To ensure timely and effective communication withthe Occupational Therapy Team Lead and other multidisciplinary Team Leads onall professional matters.
To maintain accurate documentation in line with theRoyal College of Occupational Therapy standards and CSH Surrey standards andpolicies.
To attend staff meetings and actively participateensuring understanding of information imparted.
Assess patients understanding of proposedintervention. Gain valid consent and work within a legal framework withpatients who lack capacity to consent to assessment and/or intervention.
To use a range of verbal and non-verbalcommunication skills to communicate effectively with patients in order toprogress rehabilitation and treatment programmes. This may include patients whohave difficulties in understanding or communicating patients who may bedeaf, blind, have cognitive and/or behavioural problems, or who are unable toaccept their diagnosis or presenting condition.

To participate in the appraisal scheme as anappraisee and as an appraiser for junior staff.
Participate in the supervision of students.
Deputise for the Occupational Therapy Team Leadwhen required.
To provide training andtake a role insupervision and teaching of qualified and unqualified clinical staff to includeNurses, Physiotherapists and Rehabilitation Assistants.
To be responsible for and actively record your ownpersonal development. This may include active participation in journal clubs,clinical reasoning sessions and challenging patient sessions as participant orfacilitator.
To ensure professional competence is maintained byparticipating in all Continuing Professional Development (CPD) opportunitiesand maintain a portfolio of evidence.
To participate in any training sessions as arecipient and facilitator.
To disseminate any training received to otherstaff.
