We are looking for an ANP to join our friendly and busy GP Practice. You will be part of a strong clinical team which includes six GP Partners, two salaried GPs, 2 ANPs and Practice Nursing Team.
The preferred days will be Tuesdays and Wednesdays, and this is a permanent role.
Main duties of the job
We invite applications from suitably qualified Practitioners with skills to clinically assess, diagnose, refer, and manage patients with a range of conditions.
The ability to work independently and to demonstrate critical thinking in the clinical decision-making process is required, along with the capability to take lead responsibility for the care of patients while being a strong team player.
We are a SystmOne Practice for which training would be available should this be needed.

We are a GP Practice of 11,000 patients based in the lovely spa town of Cheltenham. We are a forward-thinking, training practice with a large friendly team. A member of the local PCN, one of our GP Partners is the Clinical Director.
The wider team consists of six GP Partners, two salaried GPs, Practice Nursing Team, an HCS, two phlebotomists, admin team, care navigators, and management team.
Job responsibilities
Summary
The primary focus of the role is to work as an autonomous practitioner providing high-quality and timely patient-centred care.
The post holder will be responsible for the assessment, diagnosis, treatment, referral, and discharge of a diverse range of patients with minor injuries/minor illnesses, without reference to a doctor and within their level of competence and agreed protocols.
Primary Duties and Areas of Responsibility
Clinical
1. Make professionally autonomous decisions for which he/she is accountable.
2. Provide safe, evidence-based, cost-effective, individualised patient care.
3. Offer a holistic service to patients and their families, developing (where appropriate) an ongoing plan of care and support, with an emphasis on prevention and self-care.
4. Promote health and well-being through the use of health promotion, health education, screening, and therapeutic communication skills.
5. Undertake consultations with patients as an autonomous practitioner and use own clinical judgement to diagnose, treat, refer, and/or discharge patients.
6. Duty Practitioner Rota cover, which involves on-the-day support to patients with urgent acute medical needs.
7. Make direct referrals to secondary care and social services within locally agreed pathways, guidance, and protocols.
8. Provide clinical advice to Reception staff where required.
9. Ensure that patients receive high-quality clinical care, delivered in a timely manner.
10. Supply and administer medicines as indicated to address patient need.
11. Ensure complete and accurate documentation of each and every patient contact.
12. Work as an autonomous practitioner and as part of multi-disciplinary and multi-agency teams to ensure patients' needs are met.
13. Refer patients directly to other services/agencies as appropriate in line with Practice policy.
14. Work with the multi-disciplinary team within the Practice to promote integrated and seamless pathways of care.
15. Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.
16. Help develop and set up new patient services and participate in initiatives to improve existing patient services.
Communication and Working Relationships
1. The post-holder will be expected to communicate with staff at all levels throughout the organisation as well as patients and carers.
2. Of particular importance is the development and maintenance of effective communication and working relationships with, and between, multi-disciplinary agencies.

8. Participate in continuing professional development opportunities to ensure that up-to-date evidence-based knowledge and competence in all aspects of the role is maintained.
9. Record accurate consultation data in patients' records in accordance with the latest NMC guidance and other pertinent standards.
10. Keep up to date with pertinent health-related policy and work with the practice team and, where appropriate, with the wider locality team, to consider the impact and strategies for implementation.
11. Work collaboratively with colleagues within and external to the practice.
