Who You Are As a qualified and registered Mental Health Social Worker, you possess a robust understanding of mental health work and the statutory framework. You have experience collaborating within multidisciplinary teams and are proficient in care management. Your excellent written and verbal communication skills enable you to effectively engage with service users, their carers, and professionals. What the Job Involves Your primary role is to provide assessment, care management, and social work services within the Community Mental Health Team. You will undertake assessments of need, manage care plans, and act as a care coordinator under the Care Programme Approach. Collaborating with other disciplines, you ensure comprehensive service delivery, including safeguarding adult enquiries. Additionally, you are expected to participate in training, including becoming an Approved Mental Health Professional and Best Interest Assessor, and stay informed of relevant legislation and policies.
