Job summary
We are looking for a confident Clinical Specialist Nurse (CSN) to work within the Memory Assessment Service based in York. This is a new CSN post specifically commissioned to work with Primary care to increase the number of memory assessments and diagnosis completed within Primary care. You would work with the Memory Assessment Service leadership team and develop relationships within Primary Care and third sector organisations to establish systems and develop clinical practice to enable memory assessment and diagnosis within Primary care.
Main duties of the job
The successful post holder will provide specialist advice, guidance, training, therapeutic support, and consultation to primary care and the third sector supported by the Memory Assessment Service leadership including a Consultant Psychiatrist and Clinical Psychologist. You will provide expert advice and consultation to Primary Care teams to enable diagnosis of dementia within that setting. Due to the nature of the role, it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Due to the need to be able to support primary care teams with diagnosis and treatment, it is essential that you are a non-medical prescriber or can demonstrate that you will be able to become a non-medical prescriber within an agreed timescale. You will contribute to increasing community awareness of the presentation and prevalence of eating disorder in local populations; using this insight and intelligence to inform future investment decisions as part of community transformation and the NHS Long-Term Plan.
Qualifications
Essential
* Current Professional Registration with Nursing and Midwifery Council - RMN / RNLD
* Recognised Practice Assessor/Practice Supervisor or currently in progress of achieving.
* Trained in Clinical Supervision and providing Clinical Supervision or in the process of achieving
* Postgraduate diploma in a subject relevant to the area or be willing to work towards and achieve within 2 years. Equivalent clinical experience to that of a Post Grad Diploma/master's level such as working as a band 6 (or above) in relevant clinical area with significant experience, together with evidence of relevant CPD.
* Within the first year of being in post hold a recognised qualification in a governed therapy/advanced clinical qualification Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy/NMP (Independent Prescriber)
Knowledge
Essential
* Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally
* Understanding and working knowledge of the legal framework and statutory obligations under the Mental Health Act, Mental Capacity Act
* Detailed understanding of Safeguarding and its application in practice.
* Clinical Risk Assessment and Management and its application in practice.
* Clinical Governance and its application in practice.
* Working knowledge of clinical medicines management
* Understanding of psychological models of care and treatment (depending on specialist area)
Experience

* Significant experience working with patients with complex needs in the specialist field.
