Acute Medicine Patient Flow Co-ordinator

We are looking to recruit a Patient Flow Co-ordinator across the Acute Medicine Department at the Royal Shrewsbury Hospital.
You will be a member of the Acute Medicine Team working directly with the AMU, AMA and SDEC Nurse Coordinators (NiC) supporting the management of all attendances, admissions, and discharges for Acute Medicine patients by facilitating an efficient, effective, and timely management of patient's diagnostics, decisions, and utilization of beds and resources. You will also support ED with medical patient flow.
You will develop close working relationships with clinical and non-clinical staff ensuring they demonstrate a professional manner and personal confidence.
There is a small clinical component to this role, e.g. venepuncture, cannulation, performing ECGs.
Main Duties of the Job
1. To work directly with the NiC to ensure the safe and effective flow of patients through the department, directly supporting the Emergency Departments 4 hours patient safety standard.
2. To monitor performance in all areas of Acute Medicine, including chasing diagnostic results to support clinical decision-making and escalating unnecessary delays to the relevant departments to support patient flow.
3. To liaise with interdisciplinary teams regarding the admission and discharge arrangements of patients.
4. To ensure that Internal Professional Standards (IPS) are adhered to by contacting specialty doctors directly and escalating to NIC, Acute Physicians, and Departmental Management Team when standards are not adhered to.
5. To liaise with other agencies and carers as required to provide effective communication and thus enable timely discharge/transfer of patients out of the Acute Medicine Service.
6. To assist Registered Nurses in delivering certain aspects of health care directly to patients, within defined levels of competence and practice, while maintaining a suitable environment for care delivery.

Person Specification
Qualifications
* NVQ level 3 in Health and Social care related field, or willing to work towards one or equivalent.
* Basic computer skills.
* A good standard of numeracy and literacy - GCSE grades 9-4 (A-C equivalent).
* NVQ level 4 in Health and Social care related field, or willing to work towards one or equivalent.
Experience
* Awareness of National Access Targets.
* Good understanding of the way in which a hospital works.
* Relevant experience of working within an acute hospital environment.
