An exciting vacancy has arisen at The Beeches Medical Practice Shrewsbury, we are looking for a part time Advanced Nurse Practitioner to join our friendly team working a minimum of 18 hours per week.
Main duties of the job
The Advanced Nurse Practitioner (ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis, and treatment of patients.
The ANP provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
ANPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis, and interpretation of test results. They can independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

Overview of your organisation
The Beeches Medical Practice is a friendly 4 Partner Practice. We are situated on the south side of Shrewsbury providing medical care for approximately 7,000 people. The practice covers a wide area from Bayston Hill to Leebotwood in the north-south direction and Acton Burnell to Pulverbatch in the east-west direction.
We have two surgeries, Bayston Hill and Dorrington, Dorrington being our dispensing surgery.
We are an efficient and friendly team of 4 GP partners, 2 ANPs, 4 PN, and a HCA/Phlebotomist, a Practice Manager, Operations Manager, Dispensary Manager, Care Coordinator, and a team of 11 Medical administrators, Dispensers and Receptionists.
We are a training practice, and clinicians and staff are very involved in ensuring the trainees gain full experience of all aspects of life in general practice. We look after Medical Students from Keele University and GP Registrars for various timescales.
We use EMIS Web clinical system along with Docman and Teamnet.
Job responsibilities
The following are the core responsibilities of the Advanced Nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. Develop, implement and embed health promotion and wellbeing programmes.
2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
3. Implement and evaluate individual specialised treatment plans for chronic disease patients.
4. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health.
5. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
6. Provide specialist clinics such as contraception, etc.
7. Request pathology services as necessary.
8. Process and interpret pathology and other test results as required.
9. Deliver patient care as necessary, referring patients to secondary/specialist care as required.
10. Maintain accurate clinical records in conjunction with extant legislation.
11. Ensure SNOMED CT codes are used effectively.
12. Maintain chronic disease registers.
13. Develop, implement and embed an effective call/recall system.
14. Be responsible for the implementation of an effective immunisation programme.
15. Chaperone patients where necessary.
16. Prioritise health issues and intervene appropriately.
17. Support the team in dealing with clinical emergencies.
18. Recognise, assess and refer patients presenting with mental health needs.
19. Implement vaccination programmes for adults and children.
20. Be an extended and supplementary prescriber, adhering to extant guidance.
21. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required.
22. Contribute to practice targets (QOF, etc.), complying with local and regional guidance.
23. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.).
24. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
25. Support the clinical team with all safeguarding matters in accordance with local and national policies.
26. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
27. Deliver opportunistic health promotion where appropriate.
Person Specification
Qualifications
* a. Registered Nurse with Nursing and Midwifery Council.
* b. Post graduate diploma or degree for Advanced Practice Qualification up to December 2020.
* c. Qualified Independent Nurse Prescriber on the NMC register.
* d. Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.
* e. Meets the standards for registered ANP working at advanced level.
* f. Qualified triage nurse.
* Teaching qualification.
* ALS and PALS.
Experience
* Experience of practice within the four pillars.
* Job plan that demonstrates advanced nursing practice and has equity with peers working at this level.
* Experience of prescribing and undertaking medication reviews.
* Experience of working as a practice nurse or community nurse.
* Experience of working in a primary care environment.
