We are seeking an experienced and dedicated advanced clinical practitioner to join our team. This position is an excellent opportunity for a highly skilled and motivated individual to work remotely, on a full time basis, while making a positive impact on patient care within a supportive, collaborative environment. Main responsibilities include: Deliver high-standard, independent care, using advanced clinical skills and knowledge. Conduct patient consultations remotely via telephone, video, and Accurx, ensuring timely care and assessment. Assess, diagnose, plan, and evaluate care for acute and long-term conditions, providing expert treatment and follow-up. Prescribe medication safely in line with legislation and practice protocols, monitoring treatment effectiveness and providing patient education on medications. Take responsibility for making autonomous decisions regarding patient care, including the initiation of emergency care when necessary. Work closely with other clinical team members to ensure comprehensive care for patients, referring to specialists when necessary. Support patients in managing long-term conditions and ensure their adherence to treatment plans. Maintain accurate and up-to-date patient records in line with legal and regulatory requirements. You will have the following qualifications and experience: Registered with a UK professional body, NMC, HCPC. Master’s degree in Advanced Clinical Practice. Non-Medical Prescribing qualification. Significant experience in advanced clinical practice, ideally within primary care. Strong experience in delivering patient care, working independently, and making clinical decisions within professional boundaries. Ability to collaborate effectively with a multidisciplinary team.
