A new and exciting opportunity has arisen for an assistant pathway co-ordinator focused on end-of-life care within Oxford University Hospitals NHS Trust. The focus of the role is support the End of Life Care Lead and the clinical nurse educator to support good care at the end of life for patients dying in OUH and to support those important to the patient.
Applications are welcome from any experienced band across the Trust. This is a one-year post. If the successful applicant makes a success of the role and demonstrates positive impact, further funding could be sought, but this is not guaranteed.
Main duties of the job
The postholder will support the end of life care team to deliver patient care of high quality in OUH. Effective and efficient administration supports management through ensuring a high standard of data quality and by making the best use of capacity and resources.
The post holder has responsibility to;
Assist in the provision of a professional, comprehensive and efficient administrative service, which is effectively delivered in the EOLC team, with colleagues across OUH and key partners. This will be through the delivery of a comprehensive administrative service to the EOLC team.
·Facilitate communication and systems between EOLC team, and other teams within palliative care and the Trust.
·Keep accurate up-to-date records on EPR.
