Are you an enthusiastic physiotherapist looking to develop your clinical experience within a fast-paced community setting? This job opportunity will see you work closely with multi-organisation partners to provide rehabilitation to patients, whilst facilitating appropriate discharge destinations.
Our DTA (Discharge to Assess) Bedded Therapy Service has secured extra funding and we are looking to expand further.
The DTA Beds Therapy Service provides ongoing therapy and facilitates discharges. This includes managing caseloads across multiple care homes in South and West Hertfordshire. We also complete home visits to organise equipment for discharge home all in a timely manner with the assistance of social services. The team is part of the Unplanned Care team which includes Rapid Response and Discharge Home to Assess services and teams work collaboratively where necessary.
You will be fully supported by the team and service manager as you grow your knowledge of clinical services within West Hertfordshire and beyond to ensure patients receive the most appropriate care in a timely manner.

Undertake specialised uni-professional and multi-professional assessments.
To develop and implement treatment plans for clients. These clients will present with an exacerbation of a frailty related condition and will likely have Multiple co-morbidities including neurological conditions, musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis.
Participate in the Assessment Process when appropriate, performing overview assessments and complex specialist assessments as appropriate; including the ordering of moving and handling equipment relevant to rehabilitation need.
To communicate relevant medical information, assessment details, advice and recommendations across agencies and with clients in compliance with Trust Guidelines for sharing of information and informed consent. These agencies include General Practitioner’s, Social Services, Healthcare professionals, voluntary agencies, and residential homes.
Be responsible for gaining valid, informed consent for assessment, treatment interventions and sharing of information in compliance with legal and professional guidelines. To be able to interpret and follow these guidelines in circumstances where clients do not have capacity to provide informed consent.
