Contract: Permanent Salary: £28.971 Full time positions We are looking for staff to fit our dynamic team, we are a service for those who require short term Reablement to regain their independence whilst also supporting others to remain safely at home. Our Assessors work in the Reablement Service. The role is to assess people who are new to the service to identify what support is needed for them to remain in their homes. They then write a person centred support plan with the individual and agree upon the number of visits and the level of support the service will provide. The role also reviews the support needed by completing support visits and agreeing the longer-term goals and plans with the person. This role involves lone working and multi-agency working towards KPI targets to support both Herefordshire Council Adult Social Care and Wye Valley trust. This role involves every other weekend working Experience and Qualifications: A level 2 or 3 qualification in Health and Social Care is essential or demonstrable experience within a previous role. A minimum of 1 years, experience in Health and/ or Social Care is essential.
