Job summary An exciting opportunity has arisen to join our Primary Care Network (PCN) as CHAW PCN Health and Wellbeing Hub Manager reporting to the CHAW Operational Lead and CHAW Clinical Director. The role is for 35 hours across 5 days working out of the hub in Wimlsow Health Centre, Chapel Lane, Wilmslow. Main duties of the job The hub has been running for over 3 years now and the successful candidate would be expected to further develop the existing services of the hub in collaboration with the practices. You will be responsible for the day to day management of the team and its services. You will also act as Digital Lead for the PCN. You will have extensive IT skills, exceptional organisational skills and have worked in a patient focused environment. To take action to engage groups with low participation in chronic disease review, screening and immunisation, including childhood immunisations.
