Job summary: The post holder will take a proactive role within the clinical team to provide high standards of nursing care, disease management and health promotion, as directed by the GPs and in accordance with Practice protocols. Responsibilities include (but are not limited to): Manage Diabetic Clinics Manage routine consultations using a holistic approach Health promotion and Long Term Conditions Clinics Manage COPD/Asthma Clinics Primary Prevention Manage Vascular Clinics Understand and participate in the delivery of QOF, local and national Enhanced Services, research and recalls Initiate laboratory requests and collect specimens as required Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines, and locally agreed formula Suture removal Perform a holistic assessment of patients attending for cervical cytology smear tests (desirable but not essential, training would be given) Advise about vaccinations and injections and ensure they are administered under relevant policies and guidelines (PGDs) Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including vaccinations, medicines, safe sex, food hygiene, sun protection, etc. Recognize and manage anaphylaxis according to current UK guidelines Be able to perform cardio-pulmonary resuscitation according to current UK guidelines (updates regularly provided) Obtain and document informed consent (either verbal or written) Ensure infection control guidelines are maintained Communicate effectively with other members of the Primary Care team Preserve confidentiality of information gained at work at all times Any other duties considered relevant to the post The practice will support on going training. The post holder will be required to: Identify and manage clinical risk Assist the patient to make decisions in a style appropriate to their wishes Refer and delegate clearly and appropriately Promote and safeguard the welfare of vulnerable adults and children Participate in systems for stock and equipment monitoring and maintenance Maintain standards for infection control, hygiene and tidiness Participate in team meetings, clinical development, mandatory training and updates as required Be willing to adapt and work innovatively to support changing needs Confidentiality: In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
