Job summary Looking for an exciting role as an Occupational Therapist We are the Enfield Rapid Access Service. Our team comprises of therapists, nurses and support staff. Our aim is Urgent Care Response - hospital avoidance, and facilitated discharge. The role is community based, and you will visit patients in their homes. We offer supervision, in-service training and a range of training opportunities within the Trust. Our base is located near Enfield Town, with easy commuter links from central London/Hertfordshire and proximity to M25. A car driver is essential for this role. Main duties of the job Main Duties To work collaboratively with Primary care, Acute Services, Allied Health Professionals, Social Services, families /carers, and users of the service. To manage own caseload in terms of time management, resources, and clinical practice for the duration of the Intermediate care intervention. To undertake a comprehensive assessment of patients including those with diverse or complex presentations using a high level of clinical reasoning and assessment techniques and effectively and efficiently plan and deliver rehabilitation care programmes to patients admitted to the service. To work closely within the inter-professional team to promote independence and avoid unnecessary admission and to promote early discharge from the acute beds. To offer support, guidance, advice and teaching to patients/relatives and other carers as necessary. To participate in monitoring the effectiveness of the service and making recommendations about future needs.
