This is an exciting new ANP role, working for the Newbury Street Practice basedat Wantage Health Centre. The role will be predominately focused on same day,medically urgent patients.
Main duties of the job
Advanced nurse practitioners work autonomously within the practice usingtheir enhanced clinical assessment and treatment skills, to provide first pointof contact for patients presenting with undifferentiated, undiagnosed problemsrelating to minor illness or injury, abdominal pains, chest pains andheadaches. They are autonomous in making decisions based on assessment,diagnosis and interpretation of test results. They can independently prescribeappropriate medication, evaluate or refer to other specialists if necessary.ANP's in this role will focus solely on medically urgent patients.
This is a fiull time position and we are flexible in working hours Mon - Fri

This role will join a team of GPs, ECP, PA, Practice Nurses, NurseAssociate and HCAs. The role will be based at Newbury Street Practice.
We are based in the Health Centre, which has just been recently extended. We serve approx 16,000 patients.
There is free parking for staff.

Key duties and responsibilities
The following are the core responsibilities of the Advanced Nurse Practitioner.There may be, on occasion, a requirement to carry out other tasks; this will bedependent upon factors such as workload and staffing levels:
1. Manage patients presenting with a range of acute and chronic medicalconditions, providing subject matter expert advice
2. Implement and evaluate individual specialised treatment plans forchronic disease patients
3. Identify, manage and support patients at risk of developing long-termconditions, preventing adverse effects to the patients health
4. Provide advanced, specialist nursing care to patients as required inaccordance with clinical based evidence, NICE and the NSF
5. Undertake the collection of pathological specimens?
6. Request pathology services as necessary
7. Process and interpret pathology and other test results as required?
8. Maintain accurate clinical records in conjunction with extantlegislation
9. Ensure SNOMED CT codes are used effectively
10. Chaperone patients where necessary
11. Prioritise health issues and intervene appropriately
12. Support the team in dealing with clinical emergencies
13. Recognise, assess and refer patients presenting with mental healthneeds
14. Be an extended and supplementary prescriber, adhering to guidance
15. Support patients in the use of their prescribed medicines or overthe counter medicines (within own scope of practice), reviewing annually asrequired
16. Contribute to practice targets (QOF, etc.), complying with local andregional guidance
17. Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients, etc.)
18. Delegate clinical responsibilities appropriately (ensuring safepractice and that the task is within the scope of practice of the individual)
19. Support the clinical team with all safeguarding matters inaccordance with local and national policies
20. Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately
21. Deliver opportunistic health promotion where appropriate

4. review and process data using accurate read codes in order to ensureeasy and accurate information retrieval for monitoring and audit processes.
Supervision
The postholder will have access to appropriate clinical supervision andan appropriate named individual in the GP Practice to provide general adviceand support on a day-to-day basis.

Experience
Essential
1. Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
2. Essential clinical skills including phlebotomy, ECGs, urine analysis and swab taking.
3. Working knowledge of Microsoft and GP practice and prescribing data monitoring systems.
4. Ability to write comprehensive clinical notes, implement and evaluate care plans.
5. Be aware of data protection (GDPR) and confidentiality issues particularly within a GP practice.
Desirable
6. Mentorship or supervisory skills training.
7. Cognitive behavioural and motivational interviewing approaches / skills.

Qualifications
Essential
25. Registered Nurse with Nursing and Midwifery Council
26. Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
27. Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
28. Qualified Independent Nurse Prescriber on the NMC register
29. Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
