Under the direction of the qualified practitioner, participate in the delivery of planned care for allocated service users. The post holder will undertake a range of assigned duties which include: - Assist in the physical, emotional psychological and spiritual care of service users under the supervision of a registered nurse. Work within a team to manage risk and the implementation of the service users care plan. Provide information to enable the effectiveness of care to be evaluated. Help with the involvement of service users, and carers/relatives in the delivery of care. Provide support and assistance with relatives/carers, directing them to the appropriate professional. Assist in providing care and support to service users with a range of personal needs. These will include as directed hygiene/dressing, support with dietary and fluid intake and ensuring optimum comfort with an emphasis on a person-cantered approach. Complete physical observations, blood pressure, temperature, pulse, weight and basic tests, urinalysis, blood glucose and report the results. Assist with health promotion activities as directed. Recognise and respond appropriately to urgent medical and psychiatric emergency situations. Provide an escort to service users for specific appointments or treatment as requested by the nurse in charge. Demonstrate effective interpersonal skills when interacting with service users utilising both verbal and non-verbal communication. The use of observational skills and de-escalation techniques to avert challenging situations. Report appropriate observations and any concerns regarding service usercare to the qualified practitioner. Participation in observing and supervising service users that need continuous observations. Assist in the management of challenging behaviours utilising trust approved techniques. Participate and contribute within de briefing sessions following an incident. Ensure appropriate records are maintained under the direction of the qualified practitioner. Contribute to the development of services by sharing ideas/thoughts and giving feedback at appropriate forums. Contribute to service improvement initiatives to enhance quality of service user care. Contribute to the effective and efficient use of resources. Understand and adhere to Trust policies, procedures, and clinical guidelines. Report incidents, near misses and hazards to the nurse in charge Work with intermittent, direct supervision. The post holder is required to report to and seek the advice of a registered professional and/or Assistant Practitioner/Healthcare Support Worker with any queries.
