The delivery of clinical and therapeutic care, within the clinical setting within set protocols and within the scope of the HCSW, under the guidance and supervision of the Registered Practitioner.
Organise and prioritise time and work activities effectively.
Acts on own initiative in delivering patient care with indirect supervision.
Contributes to patient assessment, planning and delivery of nursing care ensuring the highest possible standard under supervision of Registered Practitioners.
Undertakes a range of delegated clinical care duties, records patients observations and collects specimens and samples as instructed
Ensure care is documented accurately and in a timely manner as per Trust policy and contributes to updating patients records.
Ensure that patients and relatives are treated with courtesy at all times and the individual needs of patients are respected, promoting and valuing the rights, responsibilities and diversity of people.
Promotes and participates in Health Education.
Maintain confidentiality of all patients and work related issues.

Caring for Patients

* Maintain a courteous and helpful manner at all times.
* Welcome patients, relatives and visitors to the department
* Ensure that any care or information given does not go beyond the limit of your skills, training or scope of practice.
* Answer patient queries within the limits of your training and role.
* Exchanges information with patients, relatives and other staff.
* Refer to other team members when appropriate.
* Assist with all aspects of patient hygiene needs - washing and bathing in bed, or shower, care of hair, teeth and nails.
* Assist patients to the toilet, or give urinals, bedpans or commodes. Making observations and recording of output where appropriate.
* Assist patients to mobilise, with or without the use of aids, learning and using correct methods of minimal patient handling techniques.
* Check that all necessary personal effects are within reach of the patient and that the call bell is available.
* Ensure patientâs discomfort / pain is observed and reported to registered nurse as per plan of care and hospital policies.
* Assist with the care of the dying patient and their relatives and friends, respecting all beliefs and family circumstances. Help with any final rituals/ceremonies if appropriate.
* Ensure that the patientâs property is handled in accordance with Trust procedures.
* Help in the preparation of patients for special treatments i.e. investigations, operations, etc. Ensuring all relevant documentation accompanies them, in order to maintain their safety.
* Be proactive when assisting with patient discharge making full use of any discharge services available, assisting with patient transportation when required.
* Ensure SystemOne is updated when you are admitting, moving, transferring or discharging patients.
* Be aware of the particular nursing needs of the patient, observe and report on the patientâs condition or concerns, alerting any changes/abnormalities to registered nursing staff so that any necessary treatment and/or reassurance can be given to maintain patient safety/well being.
* Transport patients to other departments within the hospital if agreed by person in charge.
* To undertake and record patient observations of temperatures, pulse, respirations, blood pressure and urinalysis as agreed by person in charge and within scope of competency
* Where appropriate assist patients to eat and drink, measuring intake and keeping accurate recordings.
* Be sensitive to ensure that patient privacy and choice is maintained, having special regard to issues of confidentiality.
* Assist with trolley preparation and ensure that the patient is comfortable.
* Undertake pressure area care using relieving practices and devices as advised by registered staff.
* Ensure that the department areas are clean and tidy, including the cleaning and restocking of equipment.
* Ensure all documentation is recorded accurately, legibly signed and dated in accordance with Trust policies and procedures.
* Demonstrate effective communication using basic computer skills to access your email account, ensuring you access this regularly and keep up to date with essential ward and Trust information.
* Be aware of current hospital procedure on Fire and Resuscitation and take appropriate action.
* Follow safe working practices and take reasonable measures for the protection of themselves and patients, following instruction from clinical staff for the welfare of patients where appropriate.
* Helps patients / relatives overcome barriers to understanding when English is not their first language.
* Reporting all complaints and untoward occurrences.
* Present a clean smart and professional appearance and adhere to ANHST Uniform Policy
* May undergo further training in ED skills which will enhance further development, such as ECG recording, wound care dressing and basic closure
