Adult Social Worker – Hospital Team BR11 (No Remote Working)

We are seeking a dedicated and compassionate Adult Social Worker to join our Hospital Team BR11. In this dynamic role, you will be an integral part of a multidisciplinary team, working directly with adults who require support during their hospital stay and transitioning back into the community. Your primary responsibility will be to assess the needs of patients, develop and implement care plans, and advocate for their wellbeing while collaborating with healthcare professionals to ensure that patients receive comprehensive and holistic care. This position requires a strong understanding of social work principles, including mental health support, community resources, and discharge planning. You will play a crucial role in navigating the complexities of hospital procedures and social services, providing crucial support to patients and their families during challenging times. The role demands adaptability, as you will be working in a fast-paced environment, often dealing with sensitive situations that require empathy and professionalism. This is a full-time position that requires on-site presence, as remote working is not an option. Responsibilities
Conduct comprehensive assessments of patients' social, emotional, and environmental needs.
Develop and implement individualized care plans in collaboration with multidisciplinary team members.
Facilitate communication between patients, families, and healthcare staff to ensure continuity of care.
Provide crisis intervention and support to patients and their families as needed.
Advocate for patients’ rights and access to necessary services and resources.
Assist with discharge planning, ensuring a smooth transition back into the community.
Maintain accurate and timely documentation of assessments, care plans, and interventions.
Requirements
A recognized degree in Social Work or equivalent qualification.
Current registration with the relevant regulatory body (e.g., Social Work England).
Experience working in a hospital or healthcare setting is preferred.
Knowledge of adult safeguarding procedures and policies.
Strong interpersonal and communication skills, both verbal and written.
Ability to work effectively within a multidisciplinary team.
Empathetic and non-judgmental approach to working with diverse populations.
Hours Per Week: 36.00
Start Time: 08:30
End Time: 17:00
Pay Per Hour: £21.00
