The Home First nursing team have two openings, Full time and Part time, for dynamic and experienced individuals to join our integrated community service. The team support in both community and secondary care settings to facilitate timely hospital discharges and enhance the quality and safety of the service given to individuals at home with an aim to optimise recovery and resilience.
The community Discharge Liaison Nurses are an adaptable team with a variety of functions to support and advocate the patient in the interface between services and organisations, identifying the appropriate pathway for the patient, ensuring 'what matters to them' is at the forefront of decisions in the process.
The service aims to deliver support for discharge to recover and assess at home over a 7 day period working in conjunction with our community multidisciplinary team and in-reaching into our hospitals across the Health board.
Main duties of the job
The role also involves that of a trusted assessor for our step down reablement provision by adopting a 'pull system' that identifies patients early on their recovery by working with ward based staff and flow teams.
The team are fundamental in discharge to assess for care home patients and initiate referrals to the Virtual ward within all participating GP practices/Clusters. This role will encompass integration with wider Health Board step down pathways whilst identifying patients suitable for virtual ward input and referral.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
