Job summary We are seeking for an Lead for the Home First Service, who is HCPC registered or has equivalent experience and registration in the field of Adult Social care, with extensive experience of Adults community and acute patient discharge pathways, the service is based at Stoke Mandeville Hospital. This post is a full time (37.5 hours a week) leadership post that involves management and oversight of the Home First Service, that supports pathway 1 discharges, from all BHT sites as well as discharges from Wexham Park Hospital for our Buckinghamshire residents. This post requires participation is the Transfer of Care Hub processes and meetings, working closely with Care Agencies, patients and their families. Successful applicants needs to have extensive knowledge of discharge supporting services, processes and a good track record of high level negotiation skills. Experience of team leadership and staff management is essential. Main duties of the job To provide delegated, day to day operational management and professional leadership the Home First service To work with the Head of Discharge and Flow, Deputy AHP Leads, discharge team, Adult Social Care colleagues, service finding team and commissioners.
