We are looking for a registered learning disabilities nurse to join our growing PCN team. You will be expected to work within your sphere of competency and within the code of conduct as set out by the Nursing and Midwifery Council (NMC)
Main duties of the job
To provide specialist nursing care and support to patients with learning disabilities within the GP Practice, ensuring access to timely and appropriate healthcare, promoting health and well-being, and working collaboratively with the wider healthcare team.
About us
Sheppey PCN is comprised of 5 GP practices situated on the Isle of Sheppey. We are looking for a registered learning disabilities nurse to work with the PCN to provide nursing care and support to our patients who have learning disabilities.
Job responsibilities
Key Responsibilities:
1. This role is accountable to the manager and you will be expected to work within your sphere of competency and within the code of conduct set out by the Nursing and Midwifery Council.
2. Assessment & Planning:
1. Conduct comprehensive assessments of patients' health needs, including physical, mental, and social aspects.
2. Develop individualised care plans, in collaboration with patients, carers, and other healthcare professionals.
3. Provide specialist advice and guidance to patients, carers, and staff on managing health conditions and promoting health and well-being.
4. Lead on Hospital Passports.
3. Clinical Care:
1. Administer medications and treatments as prescribed, adhering to relevant guidelines and protocols.
2. Provide direct nursing care, including wound care, injections, and other clinical procedures.
3. Monitor patients' health status and respond appropriately to changes in their condition.
4. Health Promotion & Education:
1. Promote healthy lifestyles and preventative healthcare measures.
2. Provide health education to patients and carers on a range of topics, including medication management, nutrition, and mental health.
3. Support patients and carers in accessing relevant services and resources.
4. Promote individuals' health and well-being, and monitor their health outcomes through effective review of the person's care plan that has been developed with them and/or their carers.
5. Work collaboratively with GPs, other healthcare professionals, and support staff to ensure seamless care.
6. Maintain clear and accurate records of patient care.
7. Participate in multidisciplinary team meetings and case conferences.
5. Advocacy & Support:
1. Act as an advocate for patients with learning disabilities, ensuring their rights and needs are met.
2. Support patients and carers in accessing appropriate services and resources.
3. Promote positive attitudes towards people with learning disabilities within the Practice.
4. Work as partners with individuals and families, providing advice and support, assessing and anticipating their healthcare needs, promoting self-care, and enabling people to be as independent and involved as possible.
5. Liaise with and support a wide range of health professionals and other partners, public, private, and third-sector agencies, to promote access to mainstream health services and reduce health inequalities.
6. Participate in an annual appraisal as well as sourcing appropriate CPD opportunities for this role.
7. Carry out work related to safeguarding as allocated and will be required to attend multi-agency meetings.
8. Take responsibility for ensuring that Complex Health Needs care plans are developed, maintained, and reviewed.
9. Contribute to clinical audit, data collection, and reporting.
10. Confidentiality & Information Security:
1. You will be required to uphold the confidentiality of all records held. This duty lasts indefinitely and will continue after you leave this post.
2. All information which identifies living individuals in whatever form (paper/picture, electronic data/images, or voice) is covered by the 1998 Data Protection Act and should be managed in accordance with this legislation. This and all other information must be held in line with NHS national standards and should only be accessed or disclosed lawfully. Failure to adhere to Information Governance policies and procedures may result in disciplinary action.

12. Governance:
1. Clinical governance is a framework through which organisations delivering health and care services are accountable to continuously improving the quality of their services and safeguarding high standards of care by creating an environment in which clinical and other forms of care flourish. Employees must be aware that clinical governance places a duty on all staff to ensure that the level of care services they deliver to patients is safe and high quality, and that they follow/comply with policies and procedures.
13. Registered Health Professional:
1. All staff who are a member of a professional body must comply with the standards of professional practice/conduct. It is the post holder's responsibility to ensure they are both familiar with, and adhere to, these requirements.
14. Risk Management/Health & Safety:
1. The post-holder has a responsibility to themselves and others in relation to managing risk, health and safety and will be required to work within the policies and procedures laid down by the company.
2. Employees must be aware of the responsibilities placed on them by the Health & Safety at Work Act (1974) to ensure that the agreed safety procedures are carried out to maintain a safe environment for other employees, patients, and visitors. It is essential to observe strict fire and security precautions at all times.
3. All staff have a responsibility to access occupational health, other staff support services, and/or any relevant others in times of need and advice.
4. All staff must report accidents, incidents, and near misses so these can be learnt from and safety improved.

* Experience in various settings, such as community, hospital, or residential setting.
* Ability to work autonomously whilst also collaborating effectively with others.
* Strong commitment to providing care that is tailored to the individuals' needs and preferences.
Qualifications
* Registered nurse with relevant experience in learning disabilities nursing.
* Knowledge of relevant legislation and guidelines related to learning disabilities.
* Excellent communication, interpersonal, and teaching skills.
* Ability to work independently and as part of a team.
* Strong organisational and time management skills.
* Experience in using electronic health records systems.
* Experience in mentoring or supporting other nurses.
* Experience working with individuals with complex physical health needs or those who engage in challenging behaviours.
* Training in areas such as mental health, end of life care, or specific learning disabilities.
* Competence in using computer systems and relevant software.
