This is aunique and rewarding role which will allow you to work with, lead and supportcolleagues during a time of innovation and development as our team focusses onthe implementation of Urgent Community response and of Virtual Wards.
You will workwithin Directorate One of Community Services, supported by senior colleagueswho are passionate and proud of the services we deliver to our community ofSouth Tyneside and Sunderland.

This vacancy is for one full time post ( hours per week) and one part time post (19 hours per week).
Main duties of the job
The team youwill work with consists of an extensive MDT which includes nurse practitioners,occupational therapists, physiotherapists, health care assistants, therapyassistant/exercise practitioners and administrative staff. The team worksclosely alongside partner agencies including those from the local and voluntarysector. This is a significant strength to our team we are proud of and supportspatient centred, integrated working.
The teamdelivers planned and unplanned care to people in their own homes, to assist inpreventing hospital admission and supporting those who have recently beendischarged from hospital, require enhanced intervention and short termrehabilitation support for their physical and functional health needs to remainat home. The service also covers rotation into Borrowdale House, a reablementunit.
The service isfast paced and you would be expected to see a wide range of patients whopresent with a variety of conditions when supporting the team clinically.
The service iscommitted to providing a sound environment for education and training tosupport the successful candidate within this role.
This role wouldinvolve working predominantly core hours of Monday to Sunday 08:00 am 20:00 pmwith flexibility to the needs of the service. We offer flexible working withinour service with a variety of shift patterns and flexibility to work until 8pmand to cover weekends.
Candidates musthave a valid UK Driving Licence.

1. To work across theservices which provide Urgent Community Response, Discharge to Assess and shortterm rehabilitation.
2. To provide care to peoplein their own homes to prevent admission/ readmission to hospital by providingrapid assessment of patients in the community over seven days.
3. Undertake comprehensiveholistic assessment of patients in both planned and unplanned situations, beable to prioritise clinical need, initiating investigations, determine adifferential diagnosis and implement a plan of care reflecting evidence basedguidance.
4. To sustain agreedstandards of care in to order to maintain integrity and dignity of all patientsin an environment which is conducive to delivering care in their usual place ofresidence or temporary place of residence including bed based rehabilitation.
5. Undertake triage withinthe service determining clinical priority of calls, working as an coordinatorensure teams are dispatched to respond to the referrals.
6. Workcollaboratively with multi professional teams to ensure practice is efficient,effective and evidence based.
7. To develop his /her scopeof professional practice in order to support the team to meet servicerequirements in accordance with professional guidelines.
8. To ensure the provisionof expert knowledge to all users of the service.
