Play a key role in improving the quality of life of adults with multiple-complex needs and disadvantage.
The Changing Futures Programme brings together a multi-disciplinary team into an established trauma informed service. We provide intensive support to vulnerable people with multiple disadvantages across Leicester city. This can include street lifestyles, offending behaviour, anti-social behaviour, substance and/or alcohol misuse, insecure housing/homelessness, mental ill-health, dual diagnosis, and women with multiple disadvantages, particularly sex workers.

This is a full-time, fixed term position working 37 hours per week from Monday to Friday. There may be occasions where you'll be required to work outside of normal office hours.
We will consider this post on a secondment basis for Leicester City Council employees only. If you wish to apply for this post on this basis, please ensure you have your line manager's approval before applying.
What You'll Be Doing
You’ll handle a case load of adults with multiple disadvantages through person-centred work and collaboration with relevant agencies. Undertaking face to face contact including on street engagement for rough sleepers, you’ll deliver support including carrying out assessments, formulating action plans and reporting outcomes. You will provide tailored support around managing health and treatment plans, establishing healthy routines, attendance at appointments, assisting with welfare benefit claims, supporting access to accommodation and sustainment of this as well as tasks such as budgeting, basic healthcare, and hygiene. Furthermore, you’ll contribute to the delivery of projects and activities within the Changing Futures Programme.
What You'll Need
With experience of providing direct support to adults with complex needs including mental health, offending, substance misuse and street lifestyles, you’ll have experience of co-ordinating and driving forward initiatives in a multi-agency environment. Confident in working with issues of conflict and confrontation, you’ll be able to diffuse and challenge difficult situations. Using excellent communication and interpersonal skills, you’ll be able to work with a wide range of people at a variety of levels, form and maintain constructive working relationships and use specialist interviewing techniques to establish a rapport with individuals and elicit sensitive information. Proactive and resilient, you’ll be persistent and persuasive with adults who may not be willing to engage with services and encourage active participation.
Appointment will be subject to Non-Police Personnel Vetting to Level 2 or equivalent.
