Job summary We are in an exciting position to be able to advertise for a community mental health nurse. We are seeking a mental health nurses who is looking to be at the forefront of future healthcare and integrated working for the older adult population, working within the Specialist Mental Health Services for Older People. As a multi-disciplinary team we provide holistic assessment and treatment pathways for older adults, and people with mental health and frailty needs. Our service provides seamless pathways between primary care and secondary care services for people with functional mental illness, and also dementia care. As a team, we can offer multi-disciplinary support from Clinical Psychologists, Consultant Psychiatrists, Occupational Therapists, Social Workers, as well as Mental Health Nurses. We also provide regular CPD sessions, reflective practice, staff support sessions, individual and group supervisions, and academic training, as well as access to external training resources.
