The Newbury Older Peoples Mental Health Service is looking for an enthusiastic, caring and highly motivatedcommunity support workerto join our friendly and supportive team.
We are a multi-disciplinary team with specialist skills in caring for older people with mental health needs and people with dementia. Therefore, we are looking for someone who has a passion for working with this client group.
In this role you willbesupportingtheco-ordination and facilitation ofevidence-basedpsychosocialgroup work for people with a diagnosis of dementia (Cognitive Stimulation Therapy), working alongsidecolleagues from psychology and the memory clinicservice.
You will also be workingalongsidecolleagues in the CommunityMental Health Team offeringmental health monitoring and supportas well assupporting and guiding patients to access and engage with communityresources.
You will receive regular individual supervision, and there will be opportunities for continuing professional development (CPD).
• Co-facilitate CognitiveStimulationTherapygroups.
• To provide support to Mental HealthProfessionalsin the provision of psychosocial assessment and intervention for behaviours that challenge in dementia.
• Support thewider servicetocompletephysical health monitoring.
• Be responsible fora caseload of patients working collaboratively with mental health professionals. Offering assessment/treatment and monitoring in line with the patients care plan.
• Provide social and emotional support to patients and carers within the context of trusting and therapeutic relationships.
• Enable the patient tomaintainand develop their social skills, encourage socialcontactsand promote social networks ifappropriate.
• Liaise with other agencies in both statutory and voluntary sectors,egGp’s, Local Authority,Alzheimer’s Society, Age Concern, etc.
• Maintain clinical records as per trust policy and clinical need.
• Participatein the process of clinicalsupervision and appraisal.
• Attend study days and training courses as agreed and consistent with the needs of the service.
• Attend team meetings asappropriate.
