The Service:
The overall purpose of Sheltered Housing is to provide the right accommodation and support when needed. In Sheltered Housing we promote independent living for residents with the knowledge that someone is always on hand to give support and help in an emergency. Residents have their own tenancy agreement with Portsmouth City Council, enabling them to live independently in their own accommodation, but with Housing-related support available as needed.
As a service, we take pride in making a positive difference in helping our residents maintain their independence in their homes. The Scheme Manager role is crucial in ensuring we can meet our purpose.
What is the role?
Residents in Sheltered Housing have a range of physical and mental health needs, requiring our support to maintain their independence. You will work to achieve this as per the Sheltered Housing purpose. You will be working as a Cat 1 Scheme Manager, providing advice and support to residents living in Cat 1 Sheltered Housing schemes in the Somerstown / Buckland / Landport area. You will be based in this area and will have an office close to the location.
Your role will include meeting and greeting new residents, introducing them to the Cat 1 Sheltered Housing Service, and making them aware of the service they can expect to receive. You will identify any support they require to live independently and take the necessary action. You will liaise with other agencies and teams to ensure that residents' needs are met.
You will act as an advocate for residents when working with other agencies and serve as a link to social services and mental health services. You will be responsible for ensuring the completion and updating of needs, risk, and support plans, ensuring that identified follow-up actions are taken. This may include offering advice and signposting to the correct agencies to help with financial issues, arranging for support with daily chores and shopping, and assisting residents in arranging repairs where relevant.
Who is the person?
You need to have:
1. Experience of working directly (face to face) with people and an understanding of the impact of physical and mental health issues experienced by older persons.
2. Good listening skills with a calm manner, able to converse easily with customers and provide advice in accurate spoken English.
3. Excellent interpersonal skills to gain the trust of residents and their families.
4. A good knowledge of the importance of confidentiality when working with residents, particularly regarding GDPR requirements.
5. A strong team player willing to support those in your team when they face challenges.
6. Understanding the need to maintain appropriate professional boundaries with residents.
7. Understanding the role of Social Services, Health Services, and voluntary agencies in providing services and being able to work with them to achieve the best outcomes for service users.
8. Good written skills to write concise notes in residents' files that accurately reflect your contact with them.
9. The ability to prioritize workloads and self-manage your time effectively to meet the different needs of each resident.
10. Confidence in lone working as well as working as part of a team.
11. The ability and motivation to promote social activities for residents.
12. Good IT skills, including the use of Microsoft Outlook and Word/Excel, and the ability to use in-house databases as required.
