We are looking for an experienced nurse to work with the Programme Manager for the West Yorkshire ICB Paediatric Palliative Care Service. You will have significant experience of working with children/young people affected by life-limiting and/or life-threatening conditions, their families and be able to evidence this.
This post will be hosted by the Leeds Childrens Hospital and your collaborative approach will enable you to work effectively with all relevant stakeholders across West Yorkshire to achieve equitable access to high quality palliative and end-of-life care for children and young people, as recommended by NICE guidance.

Main duties of the job
The focus of this role will be to design and implement an educational pathway and programme that meets the needs of healthcare professionals caring for children, young people and their families affected by life-limiting and/ or life-threatening conditions across the West Yorkshire health and care system. The post will also support the establishment of a community of practice forum to ensure that learning is also shared across the West Yorkshire system.

2. JOB PURPOSE
The Jobholder will carry continuing responsibility for a defined caseload, including the setting of standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care. The jobholder will provide advanced nursing expertise to a specific patient group and will contribute to the development of the speciality service as well as contributing to the strategic direction of nursing within the Trust. The jobholder will ensure cost effective use of resources.

Be responsible for the organisation and development of the service.
Producing business plans to support restructure of the service as/when required.
Setting out an education structure for the team
Ensure the team comply with the Trusts risk and governance agenda.
Offer specialist supervision, teaching and consultation to practitioners both within and outside the Organisation
Set up and participate in new research
