We are seeking an experienced, enthusiastic & delivery focussed project manager to help us set up and deliver Hywel Dda's Six Goals Programme. This is an innovative and exciting programme of improvement projects spanning across Urgent and Emergency, Secondary, Primary and Social Care. The Six Goals Programme is leading Wales in the development of Urgent and Emergency Care (UEC) pathways which will transform the system flows and pathways in which Health and Social Care operate. The programme will develop and implement initiatives and projects across Hywel Dda in line with Welsh Government's six policy goals for UEC:
1. Coordination, planning & support for high-risk groups
2. Signposting to the right place first time
3. Access to clinically safe alternatives to admission to hospital
4. Rapid response in a physical or mental health crisis
5. Optimal hospital care following admission
6. Homefirst approach & reduce the risk of readmission
Main duties of the job
As a Project Manager within the team you will:
1. Lead on a range of strategic service improvement projects, service developments and capital projects as part of the delivery of the Health Board's Six Goals Programme. The projects will encompass service delivery improvements, partnership initiatives, commissioning projects, including qualitative and quantitative improvements and environmental (estate) improvements.
2. Contribute to the development of the Health Board's Annual Plan/Integrated Medium Term Plan.
3. Work with Urgent Emergency Care (UEC) leads, clinicians and staff across the Health Board and wider organisations, and will promote innovation and best practice in bringing about service change in support of national priorities and drivers and responsive to local health need.
4. Provide support and advice to managers and clinicians in the Health Board on project delivery of the modernisation agenda to bring about a modernised, patient focused service.

As a project manager within the team you will be supporting the translation of vision and strategy to front line delivery, and capturing the meaningful difference to Health and Wellbeing outcomes for our population this will make. As such, we are looking for someone who can balance clear project processes and reporting with the development of positive working relationships across the system. You will demonstrate a track record of professional delivery, manage complex change projects and be able to evidence the impact of change delivered.
The Team:
You will be working in a small team consisting of Project Managers, Project Support Managers and a Programme Manager. The team is in the unique position of working directly with the Operations Directorate, but also within the Transformation Programme Office. As such, it offers a great opportunity to build profiles, knowledge and experience in both Operational and Strategic Planning settings.

* Excellent knowledge of programme and project management and capital planning and management issues.
* Project Management qualification e.g. PRINCE 2 / MSP / Agile.
* Knowledge of the NHS Business Case process.
