You will be responsible for supporting the coordination of planned/acute admissions and outpatient appointments for paediatric patients with learning disabilities and/or autistic spectrum conditions accessing services within UHS, ensuring identification and facilitation of reasonable adjustments and appropriate and effective support for patients, their families/carers as well as UHS colleagues and clinical areas.
