Key Responsibilities:
* Patient Care:
o Assess, plan, implement, and evaluate individualized care plans for patients.
o Provide direct nursing care in patients homes or community settings.
o Ensure high standards of care based on current research and best practices.
* Clinical Duties:
o Manage a delegated caseload of patients under the supervision of a senior nurse.
o Perform initial and risk assessments of patients and provide immediate care as needed.
o Maintain accurate and confidential patient records.
* Team Collaboration:
o Work closely with other healthcare professionals, including GPs, social services, and voluntary agencies, to meet the needs of the local population.
o Provide clinical advice and support to other staff members.
o Participate in the training and supervision of junior staff and students
Skills and Qualifications:
* Registered Nurse with a valid NMC (Nursing and Midwifery Council) registration.
* Strong clinical skills and the ability to work independently.
* Excellent communication and interpersonal skills.
* Basic IT skills for data collection and record-keeping.
