Job Summary
We have a fantastic opportunity for an Enhanced Clinical Practitioner (ECP) to work in the Swadlincote Acute Home Visiting service.
The Acute Home Visiting service works collaboratively with the local GP and Integrated Teams in Swadlincote, providing support to patients in their own homes and in the Oakland bedded unit, which supports patients discharged from hospital.

You will improve clinical outcomes for patients by enabling them to function independently, increasing their choice to remain in their own home/community, and reducing the need for, or preventing, inappropriate hospital admissions.

We aim to recruit clinicians who are passionate about supporting and empowering patients and their families/carers to manage their complex health needs.

We want to support patients to remain in their own homes, preventing unnecessary admission to the hospital and facilitating a return home as needed.

1. RGN or HCPC accredited degree
2. Evidence of post-registration clinical professional development

1. V300 Independent Non-Medical Prescriber
2. Clinical Decision Making course at Level 7 or equivalent
3. MA level course
4. Teaching qualification
Experience and Knowledge
Essential
1. Knowledge of practical application of clinical governance
2. High level of understanding regarding long-term conditions agenda and wider NHS agenda
3. Management of risk and health and safety, plus safeguarding adults and children procedures and ability to implement
