Job Title: Advanced Nurse Practitioner (ANP)

Healthcare Central London Ltd (HCL) is the GP Federation owned by our 31 General Practices covering the Central London (Westminster) area. The organisation supports 4 Primary Care Networks (PCNs). We operate several NHS contracts on behalf of our PCNs including a Community Dermatology Service; Community Cardiology Service and a Secondary Care Referral Service and an out-of-hospitals provision which is sub-contracted to our 31 General Practices.
On behalf of our practices we are the host employers of a large, and rapidly growing team of ARRS (Additional Roles Reimbursement scheme) roles including Clinical Pharmacists; Pharmacy Technicians; First Contact Physiotherapists; Dieticians; Paramedics; Social Prescribers; Care Coordinators, Digital & Transformation Leads, Nursing Associates, GPAs and Care Co-ordinators.

Our office-based roles are typically worked as hybrid roles the pattern of which are agreed and reviewed according to service/team requirements.
ROLE PURPOSE
* We run a range of services and our ANPs work across 3 key services:
o Additional Access weekday afternoons Monday to Friday across 6 hub locations: South Westminster Centre for Health, Lisson Grove Health Centre, Marylebone Medical Centre, Paddington Green Health Centre, St John's Wood Medical Practice and Third Floor Lanark Medical Centre.
o Enhanced Access service weekday evenings and Saturdays at South Westminster Centre for Health.
o eHub service (econsults) is a digital same-day service for our GP practices. ANPs complete telephone consultations with patients from HCL's offices at Capital House (NW1 5DH).
* We run research clinics, where we see and screen patients who are eligible for our studies.
* Furthermore, we employ Nurse Associates and ANPs may be involved in the training and supervision of these staff.
* The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
* The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
KEY TASKS AND RESPONSIBILITIES
Clinical
* Working across the following specialties; Additional Access, Enhanced Access, eHub, and clinical research.
* Triage urgent care requests if required, consulting with patients or directing them appropriately.
* Provide advanced nursing care to patients, including assessment, diagnosis, treatment, and follow-up.
* Conduct holistic assessments on patients and ensure appropriate care and action plans based on their needs are clearly recorded and documented.
* Undertake regular patient reviews.
* Plan patient discharges safely and effectively, including use of independent prescribing in respect of providing medications on discharge.
* Following assessment where appropriate/necessary and with the patients’ consent refer appropriately to other services and agencies.
* Perform and interpret diagnostic tests, such as laboratory and radiological tests.
* Prescribe and administer medications in accordance with the law and established protocols.
* Collaborate with the healthcare team, including physicians, nurses, and allied health professionals, to ensure comprehensive patient care.
* Maintain accurate patient records, including medical histories, treatment plans, and progress notes.
* Educate patients and their families about their health conditions and treatment options.
* Stay up-to-date on the latest medical research and guidelines relevant to the clinical specialty.
* Maintain compliance with all relevant clinical standards and regulations.
* Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
* Promote health and well-being using health promotion, health education, screening, and therapeutic communication skills (i.e. smoking cessation, weight management, alcohol misuse).
* Refer patients directly to other services/agencies as appropriate.
Education & Qualifications
* Current registration of Part 1 of NMC Register.
* 1st Degree/Master’s level qualification (Advanced Practitioner).
* Two years post qualification experience - consideration given to ANPs with primary healthcare experience.
* Teaching/Assessing/Mentorship Qualification.
* Independent Prescriber.
