This is an exciting opportunity to join a team providing an Integrated Disability Service for adults over the age of 25 in Conwy.
With a focus on what matters to the person, helping people acquire, re-acquire, maintain or develop their potential, the Social Worker will work to provide a service that is person centred and outcome focussed.
Working in partnership with Health and Education, the Social Worker will implement an integrated approach to meeting the social, educational and health needs of adults.
We are looking for a Social Worker with vision and passion, able to develop effective relationships and shared responsibility for supporting adults with Disabilities to live the lives that they choose.
The ability to communicate through the medium of Welsh is desirable.
