Job Summary
Care Plus Group Primary Care practices provides the first point of contact for registered patients accessing the health care service. The service will work collaboratively with patients to assist them in managing and preventing illnesses and coordinating access to specialised care.
The Care Plus Group Primary Care practices, Open Door and Quayside, provide services for a client group with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect.

Sexual Health Outreach Worker
Based at Open Door Primary Care, Albion Street, Grimsby
24 hours a week

Salary range £24,645 to £27,202, appointed on a fixed point within the range
A rare and wonderful opportunity has arisen for someone to take on the role of Sexual Health Outreach Worker who will work in conjunction with other healthcare professionals under the direction of the Clinical Lead and Operational Manager, within a range of primary care facilities and other agencies to assess and deliver a wide range of support to sex workers within North East Lincolnshire including Night outreach, sexual health screening, signposting, support and attending appointments so clients get the appropriate treatment.
To apply for this role you must have NVQ level 3 in health and social care or equivalent and have experience of working with people ideally in a health care setting.
We are looking for someone who is able to build a professional relationship with agencies and clients, can transport self and others around North East Lincolnshire and work during the day and evenings.
