CLCH Diabetes Service has an exciting opportunity for a Diabetes Specialist Nurse to join the outer boroughs of Central London Health care NHS trust.
We are looking for a diabetes nurse who is passionate about diabetes care and want to share their experiences. This role offers the opportunity for nurses with experience and skills in managing diabetes to develop their career in diabetes nursing.
The post-holders will be part of a multi-disciplinary team working with diabetes specialist nurses, diabetologists, dietitians and podiatrists and primary care health professionals.
Main duties of the job
The services provide complex care to patients with diabetes within the community - in clinics and homes. You will also be involved in delivering structured education to patients to help them improve self-management of their condition. You will be supporting GP practices by telephone/email and through mentoring and participating in case reviews or virtual clinics, face to face clinics, held with GPs, Diabetes Consultants and other HCPs.
As well as the clinical delivery, you may be required to lead on various service development projects such as audits, patient stories and patient engagement. In addition, you will be contributing to upskilling other health care professionals to provide a high standard of diabetes care.
We are looking for a nurse with reasonable experience of Diabetes care and passionate about working with adult patients with Diabetes in the Community. Job responsibilities
1. The Diabetes Specialist Nurse (DSN) has a responsibility for the development and management of the caseload and also facilitating DESMOND education with other members of the team. The DSN also partake in MDT meetings and where indicated undertake home visits. Ensure ongoing practice development and research in line with the National Service Frameworks, government, regional and Trust initiatives.
2. The DSN will work in close collaboration with medical and other multi-disciplinary staff to provide a comprehensive patient focused service for individuals with complex Type 2 diabetes.

1. Registered Nurse with current UK NMC registration
2. ENB 928 or Warwick Certificate in Diabetes Care (CIDC) or equivalent

2. Experience in Diabetes
