In the Bromsgrove (NMHT), we strive to improve the mental health and wellbeing of our local communities.
We have incorporated a huddle into the daily functioning of the team, which enables us to be focused on the recovery of the patient through a multidisciplinary approach, with expertise from doctors, nurses, occupational therapists, support workers, and psychological therapies staff.

Main duties of the job
1. Will work as a key member and resource of the MDT core team.
2. Close working within the MDT, undertaking and supporting individuals accessing the service in Recovery focused activities.
3. Scope for professional development as the trust has a repertoire of training available to those wishing to extend their skill set.
4. Support registered professionals to deliver evidence-based interventions, managing a caseload of patients.
5. Support and signposting to improve social isolation and improved population health.
6. You will have experience of working with this user group in community settings.
7. The ability to work flexibly as the need of the service demands is essential.
8. Attend weekly MDT meetings, participate in professionals meetings, signpost to adjunct services depending on patient need.
9. Support and guidance to the carers of patients accessing the services.

Job responsibilities
1. Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism.
2. Use their initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.
3. Work across all clinically appropriate patients, 17-years-old and over, to deliver evidence-based interventions.
4. Provide support to MDT colleagues in delivering interventions.
5. Work as an integral team member, bolstering recovery-based interventions in the community.
6. Support medicines management/clinics related to physical health delivery.
7. Represent the team on specific projects and working groups as required.

Person Specification
Qualifications
1. Essential: Level 3 qualification (A-level, NVQ3) in Care or Level 2 qualification (GCSE, NVQ2) in care and will be expected to undertake the relevant level 3 qualification on appointment to post.
2. Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust.
3. I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
Additional Criteria
1. Essential: Able to meet the travel requirements of the post.
Skills and Abilities
1. Essential: Ability to communicate clearly and effectively in English in both verbal and written formats.
Experience
1. Essential: Previous relevant experience working with the client group.
