What the Job Involves In this role, you will provide comprehensive support to service users, working collaboratively to design and implement reablement plans. You will assist in establishing effective relationships and deliver practical assistance to meet service users' needs. Your key responsibilities will include assessing, planning, and supporting care plans, promoting the inclusion of service users, and working with various professionals and stakeholders. You will contribute to training efforts, ensure compliance with Council policies, and maintain accurate records to support service delivery. Skills Excellent communication skills in spoken and written English Experience in adult social care and supporting vulnerable adults Proficient in IT systems, including Excel and Word Ability to conduct risk and medication assessments Strong organizational skills and ability to prioritize workload Ability to work independently and as part of a team Full driver's license and access to a vehicle NVQ/QCF Level 3 in Adult Social Care or equivalent Trusted Assessor training (desirable)
