Tollgate Medical Centre is one of three practices that form Docklands PCN. Individually, we provide Primary Care Services to around 18,500 patients, and our PCN provides care and support to over 40,000.
We are a strong practice, and are proud of the work we do within our PCN. We have an expanding multidisciplinary team including GPs, Nurses, HCA, Pharmacists, Social Prescribers, Care Coordinators, Paramedic, First contact Physiotherapists, and others. Job responsibilities
The post holder is an experienced general practice pharmacist, who acts within their professional boundaries, supported and working alongside the team at Tollgate Medical Centre.
Working as part of a multi-disciplinary team, the post holder will provide clinical pharmacy leadership for the practice, taking responsibility for, and not limited to, Structured Medication Reviews, areas of chronic disease management, medicines optimisation, quality improvement, the quality and outcomes framework and other enhanced services.
The post holder will be responsible for leading, training, and coordinating the work of professionals within the organisation, to proactively and competently manage repeat prescribing systems, proactively dealing with acute and repeat prescription requests and medicines reconciliation on transfer of care. Where necessary, the role would involve putting in place systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice.
The post holder will provide support to the practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage GP workload.
You will work closely with colleagues within the Practice and the PCN to ensure adherence to prescribing formularies, and medicines optimisation programmes, whilst maintaining patient safety. Regular meetings within the PCN, community pharmacists, and the ICB Pharmacy team are essential to share learning and to be up-to-date with new guidelines and targets.
The role is pivotal to improving quality of care and operational efficiencies and so requires motivation and passion to deliver excellent service within general practice and the wider PCN.
