Chapeltown Primary Care Network (PCN) have an exciting opportunity for a full or part time Clinical Pharmacist; working within the pharmacy team as well as the wider PCN and practice teams at Allerton and Westfield Medical Centre, St Martins Practice and Woodhouse Medical Practice.
