Saints Foundation bring Southampton FC to the heart of the community, using the power of the club and our passionate team to help fulfil individual potential and make Southampton a better place to live for all. The role will support the falls recovery team to ensure that all transport needs for the service are met. The role will enthuse staff and beneficiaries alike, creating opportunities to improve participants’ health outcomes and life choices, whilst delivering a high-quality support and transport service. You will be directly responsible for driving the project bus and following present routes and schedules to ensure all participants are delivered and collected from their scheduled sessions. The role will work closely alongside the Falls Recovery support role to ensure all elements of the participants transport, and sessional needs are met, whilst working closely with all staff and volunteers to ensure that they have the tools and support they need to ensure that services are delivered are ‘Best in Class’. Key Responsibilities: Provide transport for older adults referred on to the Falls Recovery programme to and from their homes to a variety of community venues in and around Southampton. Help to plan and coordinate transport routes to ensure effective use of time and resources. Work closely with the Falls Recovery team with regards to any changes to the weekly schedule and in supporting the behaviour and welfare of the participants. You will be responsible for the welfare of vulnerable adults in your care. You will be responsible for the health and safety of vulnerable adults in your care including ensuring safe practice when driving, collecting participants from homes or agreed locations. Work with support officer to schedule transport and pick up routes, ensuring comfort and security for participants, whilst considering the most cost effective and time efficient process. Support delivery staff at all Falls sessions to provide extra support to referred patients as required. Ensure all patients that require transport support are contacted regularly and included within the phone call support list where appropriate. To provide support, advice and guidance to Health & Wellbeing casual staff members and volunteers, as directed by the Project Coordinator. Work with staff across the health Pillar to develop innovate new solutions and project ideas. Essential Experience: Experience of driving and transporting community groups. Desirable Experience: Have a good understanding of the key Health & Wellbeing services in Southampton and be able to signpost service users accordingly. Experience working with adults with health conditions. Experience working with older adults. Essential Qualifications (including certifications): Hold a clean, valid driver’s license (passenger carrying vehicle) entitlement (D1). Desirable Qualifications (including certifications): Dementia awareness. Safeguarding qualification. Emergency Aid certification Essential Skills: Good communication and organisation skills. Calm and stable temperament. Ability to establish and maintain good relationships with people from a range of backgrounds. Confident and outgoing personality. Positive Attitude Ability to work on own initiative. Be able to work as part of a multi-disciplinary team. Good knowledge of the local area. Good Organisational skills. Desirable Skills: Good knowledge of vehicle maintenance Excellent attention to detail. Ability to engage with hard-to-reach groups.
