SS9 Primary Care Network has an exciting opportunity for a hardworking and enthusiastic emergency care practitioner (ECP) to join our PCN, on a full-time basis for 37.5 hours per week, including a late shift until 8pm and some Saturdays. The ECP role is part of the home visiting service which aims to support patients to receive treatment and care at home, where possible, and reduce unnecessary hospital admissions. You will need to listen to patients, their families and friends and work collaboratively to develop effective packages of care for people. Within the team the ECP will focus on the acutely unwell patients who may not require a hospital admission with prompt assessment and treatment at home. The ECP will triage, take a comprehensive history, examine, diagnose and treat/refer for a range of minor injuries/illnesses/infections as required. You are expected to work autonomously but will have the support of a multi-professional primary care team made up of doctors, nurses, advanced nurse practitioners and pharmacists. About the Candidate You will have the following skills and experience: Essential: Be a nurse, paramedic or allied health professional with experience in an emergency care setting. Have m embership of a professional body e.g. RCN. Hold a Nurse Practitioner Degree, BSc in Emergency Care or equivalent. Be an independent nurse prescriber. Have consultation skills training. Experience using clinical examination and assessment skills within practice. Experience of minor illness/triage, A&E or as an ECP. Experience in telephone triage. Primary care experience with both adults and children. Excellent communication, and organisational skills. Demonstrable leadership skills. Desirable: Qualification in advanced practice or currently working towards. Teaching/mentorship qualification. Experience of nursing people in their own homes. Good working knowledge of SystmOne.
