The team will provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that the service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided. The team will deliver home treatment 24/7 to an identified group of children and young people in Crisis by responding quickly to those referred with urgent and high-risk mental health needs, and provide collaborative, effective, evidence based interventions at the right time with the right person. This team will work within a multi-disciplinary framework, and work closely with other pathways and colleagues to ensure smooth transitions take place, including transitions to adult services or alternative specialist providers and support for and facilitation of early discharge from hospital.
There will be scope for creativity in this team, to develop evidence based interventions, to think systemically and work collaboratively with children, young people and families, informed by the local Transformation Plan, Future in Mind and the THRIVE framework. Main duties of the job
Band 5 nurses have a crucial role in delivering safe and effective home treatment for an identified client group and in accordance with that persons' wellbeing and recovery action plan. The post holder will work directly with children and young people with mental health needs and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with the aim of helping children and young people participate in normal family and community activities and to facilitate positive outcomes of care in relation to young people's mental health needs.
You will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved. You will assist in the delivery of a high standard of nursing care to service users referred to the team. You will implement agreed systems for the monitoring of effectiveness of care. You will participate in the education and management of junior staff.
Job responsibilities
1. Contribution to the delivery of home treatment through nursing care to an identified community client group and within a multi-disciplinary team.
2. Support for and facilitation of, early discharge from hospital.
3. This is a 24 hour service which will involve shifts including weekends and nights.

6. To be responsible for an agreed caseload to be determined by the operational lead and reviewed under Clinical supervision.
7. To provide mental health assessments within a defined timescale.
8. To implement effective therapeutic interventions utilising evidence-based practice, within the framework of a Bee U. Using a structured and agreed approach through short term intervention that will meet the needs of the service user.
9. The post holder will liaise with professionals from external agencies and organisations in order to maximise partnership working opportunities.
10. To be able to work autonomously managing risk to patient safety and developing robust care plans, effective risk assessment and risk management plans.
11. To offer innovative clinical solutions to enable most effective use of resources for the benefit of patients/public/users across your service.
12. To be collaboratively responsible for the delivery of high quality, effective and efficient provision and work in collaboration with professional colleagues.
13. To ensure high standards of practice to current and new evidence of effectiveness, including NICE guidelines. To contribute to the development and implementation of policies and practice guidelines for the multidisciplinary team.
14. To support the team in reviewing, auditing, monitoring, evaluation and researching care delivery in line with NSFs, local and national directives.
15. To provide evidence and assurance to the Clinical Service Manager that clinical standards are established and being consistently met.
16. Organising and prioritising own workload on a daily basis.
17. To implement and help evaluate interventions, utilising a range of resources in order to achieve positive outcomes.
18. Implement and support detailed care plans, outcome measures documentation, risk assessments and treatment programmes (including mental health and multi-disciplinary care plans).
