You will be responsible for providing mental health triage, assessment and outcomes-based intervention to meet service users' needs within the Primary Care Network. You will be responsible for identifying need and best course of advice, support or intervention, promoting psychosocial wellbeing by identifying and then supporting pathways for service users and carers.
