1. The post-holder will be responsible for the holistic care of children with complex health care needs in the childs own home and special school. 2. Develop and maintain clinical competence and skills by undertaking a competency based training programme, with assessment and re-assessment at regular intervals 3. Carry out all relevant forms of care without direct supervision following care plans and the trust policies and procedures 4. To observe patients and respond to changes in the childs condition, including emergency interventions. Verbally report changes in their condition to appropriate members of staff and document in nursing notes. 5. To provide child specific care to support the clinical needs of the child in the childs home or community as required. This may include tracheostomy care, enteral feeding support, maintaining airways by using suction techniques., catheterisations. This list is not exhaustive. The clinical care will be child specific following competency based assessments 6. Recognise the cultural needs of patients and staff and enable them to be fulfilled. 7. Accompany child/young person to designated community place i.e school or hospice if BCPHT are supporting a package of care 8. Following training and clinical competencies recognise when the childs condition deteriorates and act appropriately 9. Under direction, support with continence product reassessments. 10. Support children and families with seizure management including administration of emergency rescue medication, following an agreed care plan and maintaining competences 11. Following direction from the nurses-administer minor medication as per care plan/policies and as agreed by the RCN 12. To ensure equipment is maintained and faults are reported following trust policies and competencies are up-to-date in the use of the equipment 13. Deliver equipment and consumables as required 14. To participate in administration of file notes, filing letters, typing and general administration duties as requested by the team leader 15. To participate in the ordering/stocking and stock control of equipment 16. Promote health, comfort, dignity, privacy and a quality of life whilst supporting and promoting the Childs and families rights. 17. To work within the complex/continuing care framework and protocols providing and maintaining a high standard of care to patients as delegated 18. Participate in the planning, implementation and evaluation of care under the direct/indirect supervision of a qualified or registered Nurse. 19. Work in partnership with the multidisciplinary team according to the Care Plan under the guidance and delegation of appropriate professionals. 20. Communicate effectively with the child and their family acting as an advocate when needed. 21. Maintain clinical records using Trust documentation policy. 22. Communicate effectively with all professionals to provide a seamless service. 23. Respect the child and young person and families individual needs. 24. Understand age related developmental needs of the child, young adult. 25. Help promote and maintain a suitable environment in which the child, young person feels safe and comfortable whilst promoting usual daily activities. 26. Recognise the parents knowledge and expertise of their child, young adults care needs. 27. Respect the child and families right to privacy whilst a guest in their home. 28. Participate in an initial introductory and ongoing competence based training program according to the childs changing needs.
