Job summary An exciting opportunity has arisen for a Clinical Neighbourhood Matron to join Northumbria Healthcare'sdynamic Community Nursing Service working across a cluster of practices within a defined geographical area. In addition, the post holder will lead and be responsible for District Nursing teams within the neighbourhood by developing new effective ways of working. Main duties of the job The post holder will be responsible for operational management and clinical leadership across a primary care network working directly to the Senior Manager, maintaining a high standard of care delivery to patients, clients and relatives. The post holder will be required to represent the Community Nursing service, working with other health and social care providers to shapean integrated whole system approach to service delivery across the primary care network. The post holder will work towards advanced professional practice including advanced clinical skills, medicines management, leadership, supporting self-care management and forge interagency and partnership working to improve outcomes for patients with co-existing long term conditions. In addition, the post holder will work proactively to reduce unplanned hospitaladmissions and facilitate earlier discharge from hospital.
