An exciting job role has recently become available at the Lancashire and South Cumbria NHS Foundation Trust. They are looking for a new member for their Community Mental Health Team.
The roles’ purpose is to work within their professional scope of practice to actively promote values-based practice when working with others to ensure rights-based and recovery focused approaches whilst advocating for the service user.
Requirements:
* Complete comprehensive assessments and personalised care which is dynamic and flexible to support service users and their families/carers.
* Provide evidence based interventions and providing specialist advice
* Work as a role model within a multidisciplinary team enhancing the unique value of mental health practitioner.
* Deliver evidence based interventions following NICE guidelines to promote recovery.
* Regularly review care and outcomes, supporting service users to complete clinical outcome.
* Measure and monitor progress and adapting treatment options as require.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
