Responsibilities
Supervise and lead a team of support workers in delivering high-quality care and support to clients.
Develop, implement, and oversee personalized care plans tailored to individual client needs.
Conduct regular assessments of client progress and adjust care plans as necessary.
Provide training and mentorship to support workers, fostering skill development and team cohesion.
Maintain accurate records and documentation of client interactions, plans, and progress reports.
Collaborate with multidisciplinary teams to ensure comprehensive care for clients.
Act as a key point of contact for families and external agencies, advocating for clients' needs and rights.
Requirements
Proven experience in a support worker role, preferably at a senior level.
Strong understanding of the principles of person-centered care and support.
Excellent leadership and supervisory skills, with the ability to motivate and develop a team.
Effective communication skills, both verbal and written, with the ability to engage with a diverse range of individuals.
Relevant qualifications in health and social care or equivalent experience.
Demonstrated ability to manage challenging behaviors and crisis situations effectively.
Commitment to ongoing professional development and adherence to best practice in care delivery.

Pay Per Hour: £12.47
