We are looking for an enthusiastic and pro-active AdvancedNurse Practitioner withexcellent clinical and communication skills to join our team. The suitablecandidate will be an independent prescriber who is able to work independentlyand manage their own workload. The rolewill be part time and include assessment and treatment of minor ailments withinthe practice and care home visits.
We are afriendly and forward thinking multi-disciplinary GP practice and prideourselves on the provision of high quality care. Our clinical team comprises of 3 GP Partners, Salaried GPs, a prescribing Pharmacist and excellent Nursing team.
Main duties of the job
Responsible for providing high quality care to patients. Clinically assessing, diagnosing and providing treatment to patients
Provide face to face and telephoneconsultations for patients presenting with minor ailments and chronic problems,to establish a diagnosis and management plan using history taking, physicalexamination, problem solving and clinical decision making
Communicating effectively with patients, carers and all members of staff within the practice
Manage acute and chronic medical conditions. Provide care home visits and visits for housebound patients toassess and provide appropriate management of acute and chronic conditions
To prescribe safe, effective and appropriatemedication as defined by current guidelines and within own professionalcompetencies
Opportunistic health promotion, particularlyin relation to diet, exercise, smoking and alcohol and providing theappropriate preventative health advice and referrals where necessary
Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
Knowledge of local and national policies, protocols and guidelines

Job responsibilities
Provide face to face and telephone consultations for patients presenting with minor ailments and chronic diseases, to establish a diagnosis and management plan using history taking, physical examination, problem solving and clinical decision making
Provide home visits to housebound and care home patients to assess and provide appropriate management of acute and chronic conditions
Assist the clinicians with shared admin such as workflow, tasks and the review and action of pathology results
To prescribe safe, effective and appropriate medication as defined by current guidelines and within own professional competencies
Provide care and treatment to patients with chronic diseases through pro-active review and management of the condition focusing closely on individual management to improve health care and compliance
Opportunistic health promotion, particularly in relation to diet, exercise, smoking and alcohol. Providing the appropriate preventative health advice and referrals where necessary
Provide general and specific health screening within agreed protocols with referral to relevant departments where necessary
Work with patients in order to support compliance, and adherence to treatments providing information and advice on prescribed or over-the-counter medication on regimens, side-effects and interactions
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
Assist with vaccination campaigns providing appropriate and timely vaccinations including flu, shingles and covid vaccinations

* Experience of working within a nursing team.
* Experience of triaging, diagnosing and treating patients.

Qualifications
* Registered Nurse at degree level
