R&V Ltd is looking for individuals with an upbeat personality who are ready to work as care assistants for our client. We are seeking passionate and dedicated professionals to join our team. It is essential that candidates are patient, understanding, and respectful towards the residents' individual abilities, and owning a car is necessary.
The requirement of the hospital Social Work Team is to respond within timescales to assess people both in the hospital and those who have recently been discharged. We assist them with arranging any care and support they may need in the longer term to maximize their independence. The work is varied, fast-paced, and there are opportunities to gain experience working with a variety of professionals both in the Hospitals and Community Resources, including the Voluntary sector.
Your role will include:
1. Accountable for delivering a statutory social work service.

3. Working in partnership with colleagues to deliver a high standard of social work practice.
4. Supporting people to regain their skills and independence following their hospital admission wherever possible.
5. Having access to a strong and dynamic social work service with clear leadership.
Experience and Key Skills:
1. Experience of working proactively as a member of the team to make an accurate strengths-based assessment of needs and negotiate and implement effective plans and outcomes (in line with relevant legislation), working in partnership with carers, colleagues, and a variety of external agencies to meet people's outcomes.
2. Experience of applying a risk enablement approach and using evidence-based practice.
3. Experience of analysis and interpretation of people-related information and data to prioritize needs and formulate plans for intervention and service provision.
4. Experience of contributing to or undertaking safeguarding inquiries.
5. Ability to provide informal supervision and develop the capabilities of less experienced social care practitioners, and to mentor and coach students and apprentices.
6. Excellent communication and interpersonal skills to interact effectively with a range of stakeholders to provide clear advice and guidance and interventions.
7. Ability to recognize the limits of own accountability and responsibility, know when to seek appropriate support, advice, and supervision, and to prioritize and organize work to meet agreed performance expectations.
8. Sound and accurate IT skills to ensure effective record keeping, compliant with data protection and respecting confidentiality.

* National insurance number
* Right to work in UK
* Previous experience is essential: minimum 2 years as a Qualified Social Worker
* Health and social care relevant training
* Must be able to drive and have access to their own car in order to commute to and from the service
* Manual Handling Practical training
* Available to be in hospital 2-3 days per week
