Swan Surgery in the beautiful, historic town of Bury St Edmunds has a fantastic opportunity for an experienced practice nurse to join our team on a full or part-time basis, depending on the successful applicant. The main responsibilities of the role include: Undertaking health assessments, administering medication, treatments & performing diagnostic tests. Providing & evaluating a range of health interventions to ensure that patients’ health & social care needs are met. General health & well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing & treating long-term conditions, i n line with national/local policies & the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems & intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Implementing, & participating in, vaccination/immunisation programmes for both adults & children, including patients travelling abroad. Meeting the needs of patients presenting for opportunistic wound care. Hypertension review & management. Running asthma, COPD, diabetes clinics & reviews. New patient & health checks. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: Current NMC registration with an active PIN. Experience of working in primary care (desired but not essential) Experience of working as a practice or community nurse (desired but not essential) Experience of chronic disease management. Experience in wound care, ECGs, venepuncture, women's/men's health, immunisations, new patient registrations & requesting pathology results. Ability to record accurate clinical notes. Excellent interpersonal skills. Ability to follow clinical procedure. Strong IT skills.
