Requirement for; Occupational Therapist – Rapid Response - Taunton / Bridgwater area

Hours: Full time or part time (min 3 days a week)
Length: 5 months
Grade: Band 6 (would consider experienced Band 5)
Location: Rapid response /UCR service – Taunton / Bridgwater area
Notes: We require someone who can work in the community to rapidly respond to patients' needs - being flexible and adaptable as no 2 days are the same. The rapid/UCR service provides urgent responses within 2 hours or same day to patients who have gone into crisis in order to prevent hospital admission or quick turnaround from the A&E department. Candidates need to be flexible and adaptable to the work streams they are required in.
Requirements:
* Car owner and driver essential.
* Skills and knowledge within community working and/or acute services.
* A broad range of skills and experience within care of the elderly, medical, and complex case management.
* Ability to work remotely and as part of a team, being flexible with caseload management.
* Work will also involve end-of-life care.
* Ideally looking for candidates from the West side of Somerset, covering the Taunton/Bridgwater area, with the expectation of covering a wider patch as service needs arise.
